Where do elected officials and appointed officers file an FDS form?

Effective January 2023, elected officials and appointed officers are required to file an original FDS using the County of Maui electronic filing system.  The form will be transmitted and filed automatically with the Board of Ethics after submittal; a copy of their FDS will be transmitted via email to the Office of the County Clerk (7th Floor, County Building).  Elected officials, appointed officers, and candidates will receive an electronic email confirming receipt of their FDS from the Board of Ethics and the Office of the County Clerk.  Should there be any issues with an FDS submission, the filer will be contacted individually by a staff member from the Board of Ethics to rectify and to resubmit if necessary.
